Analisis Drug Related Problems Pasien Depresi di Rumah Sakit Jiwa Marzoeki Mahdi Bogor Triwulan I 2024

Masalah terkait obat lazim terjadi pada pengobatan jiwa termasuk depresi yang dapat menyebabkan morbiditas dan mortalitas pasien. Tujuan penelitian ini untuk menganalisis kejadian DRP pada pasien depresi. Penelitian ini merupakan studi cross sectional dengan pengambilan data secara prospektif. Subyek penelitian adalah seluruh pasien depresi di poliklinik Cemas & Depresi periode Januari sampai Maret 2024 yang memenuhi kriteria inklusi. Pada penelitian ini dilakukan analisis faktor yang diprediksi berpotensi menyebabkan DRP terdiri dari karakteristik demografi dan klinis menggunakan uji regresi logistik. Analisis DRP menggunakan Pharmaceutical Care Network Europe (PCNE) V9.1. Jumlah sampel penelitian sebanyak 173 pasien. Pada penelitian ini pasien jenis kelamin terbanyak yaitu perempuan (68,2%); rata-rata usia 38,4 tahun; dengan pendidikan tinggi (72,8%) dan tidak bekerja (57,2%). Lama pengobatan rata-rata 22 bulan; jumlah obat rata-rata 3 item; dan tanpa penyakit penyerta (86.1%). Antidepresan yang paling sering diresepkan adalah sertralin (62,9%) dan sebanyak 91% merupakan terapi kombinasi dengan kombinasi terbanyak adalah sertralin-clobazam-lorazepam (22%). Terdapat 655 DRP dengan 194 DRP aktual (29,6%) dan 461 DRP Potensial (70,4%). Secara keseluruhan DRP paling banyak terkait potensi interaksi obat sebanyak 59%; durasi pengobatan terlalu lama sebanyak 21,2%; terkait kepatuhan minum obat sebanyak 8,4%; dosis obat terlalu rendah sebanyak 6,1% dan penyebab lain sebanyak 5,3%. Antidepresan yang sering menyebabkan DRP adalah amitriptilin 35,7%. Obat yang sering menyebabkan DRP adalah lorazepam sebanyak 36,4% dan clobazam 33,8%. Karakteristik pasien yang berpengaruh signifikan (p value < 0,05) terhadap kejadian DRP adalah usia (p value 0,035; odds ratio 1,044; 95% CI 1,003-1,087); jumlah obat (p value 0,000; odds ratio 3,206; 95% CI 1,951-5,268) dan lama pengobatan (p value 0,007; odds ratio 1,035; 95% CI 1,009-1,061).
